Altruist Home Care Solutions is seeking a Home Health Nurse RN to provide comprehensive nursing care to patients in the home setting within the Dallas-Fort Worth and East Texas region. You will manage a caseload of patients, perform clinical assessments, implement treatment plans, and coordinate care with the interdisciplinary team. This role requires travel within a 30-mile radius of the corporate office located at 12660 Coit Rd. Suite 200, Dallas, Texas 75251.
Responsibilities
- Assess and evaluate patient cases and prepare and implement treatment and case management plans
- Perform accurate OASIS collection and ensure medication profiles remain current
- Ensure lab values are reported to the physician in a timely manner
- Attend weekly case conferences and case manager meetings
- Prepare and maintain written case records, reports, and forms
- Confer with health team members to exchange, discuss, and evaluate patient information for planning and coordinating treatment programs
- Explain procedures and treatments to patients to gain cooperation and understanding
- Administer prescribed medications and treatments in accordance with physician orders and approved nursing techniques
- Maintain awareness of comfort and safety needs of patients
- Observe patients, record significant conditions and reactions, and notify supervisor and/or physician of patient condition and reactions to drugs, treatments, and significant incidents
- Take temperature, pulse, blood pressure, and other vital signs to detect deviations from normal
- Respond to life-saving situations based upon nursing standards, policies, procedures, and protocol
- Document nursing history and physical assessment for assigned patients
- Initiate patient education plans according to individualized patient needs as prescribed by physician and/or company policy
- Participate in quality improvement activities
- Complete all documentation within 24 hours of visits, 7 days per week
Required Qualifications
- Current and valid Texas RN license
- Minimum 2 years of nursing experience
- Minimum 1-2 years of hospice experience
- Current and valid Texas driver's license
- Proof of current and valid auto insurance
- Proof of TB skin test result
- Impeccable work history with professional references
- Willingness to see patients in the assigned coverage area
- Proficiency with OASIS admissions for home health patients
- Current CPR certification
- Personally owned automobile that is insured in accordance with state and organization requirements and is in good working order
- Ability to commute to the Dallas office and be onsite (this is not a hybrid role)
Preferred Qualifications
- 1-2 years of home health experience
- Working knowledge of various EMR systems such as Careficient, Winnolas, or HomeCare Homebase
Education
- Bachelor's degree in healthcare administration, nursing, or a related field
- Strong understanding of healthcare terminology, regulations, and practices
Skills and Abilities
- Excellent verbal and written communication skills
- Strong organizational skills
- Excellent interpersonal skills and ability to communicate effectively with the Altruist team, patients, and referral sources
