The opportunity

Job Description

Altruist Home Care Solutions is seeking a Home Health Nurse RN to provide comprehensive nursing care to patients in the home setting within the Dallas-Fort Worth and East Texas region. You will manage a caseload of patients, perform clinical assessments, implement treatment plans, and coordinate care with the interdisciplinary team. This role requires travel within a 30-mile radius of the corporate office located at 12660 Coit Rd. Suite 200, Dallas, Texas 75251.

Responsibilities

  • Assess and evaluate patient cases and prepare and implement treatment and case management plans
  • Perform accurate OASIS collection and ensure medication profiles remain current
  • Ensure lab values are reported to the physician in a timely manner
  • Attend weekly case conferences and case manager meetings
  • Prepare and maintain written case records, reports, and forms
  • Confer with health team members to exchange, discuss, and evaluate patient information for planning and coordinating treatment programs
  • Explain procedures and treatments to patients to gain cooperation and understanding
  • Administer prescribed medications and treatments in accordance with physician orders and approved nursing techniques
  • Maintain awareness of comfort and safety needs of patients
  • Observe patients, record significant conditions and reactions, and notify supervisor and/or physician of patient condition and reactions to drugs, treatments, and significant incidents
  • Take temperature, pulse, blood pressure, and other vital signs to detect deviations from normal
  • Respond to life-saving situations based upon nursing standards, policies, procedures, and protocol
  • Document nursing history and physical assessment for assigned patients
  • Initiate patient education plans according to individualized patient needs as prescribed by physician and/or company policy
  • Participate in quality improvement activities
  • Complete all documentation within 24 hours of visits, 7 days per week

Required Qualifications

  • Current and valid Texas RN license
  • Minimum 2 years of nursing experience
  • Minimum 1-2 years of hospice experience
  • Current and valid Texas driver's license
  • Proof of current and valid auto insurance
  • Proof of TB skin test result
  • Impeccable work history with professional references
  • Willingness to see patients in the assigned coverage area
  • Proficiency with OASIS admissions for home health patients
  • Current CPR certification
  • Personally owned automobile that is insured in accordance with state and organization requirements and is in good working order
  • Ability to commute to the Dallas office and be onsite (this is not a hybrid role)

Preferred Qualifications

  • 1-2 years of home health experience
  • Working knowledge of various EMR systems such as Careficient, Winnolas, or HomeCare Homebase

Education

  • Bachelor's degree in healthcare administration, nursing, or a related field
  • Strong understanding of healthcare terminology, regulations, and practices

  • Excellent verbal and written communication skills
  • Strong organizational skills
  • Excellent interpersonal skills and ability to communicate effectively with the Altruist team, patients, and referral sources
  • Compliance with accepted professional standards and practice
