The opportunity

Job Description

Altruist Home Care Solutions is seeking a Registered Nurse to provide comprehensive home health and hospice care to patients in the Dallas-Fort Worth and East Texas region. In this role, you will manage a caseload of patients, delivering skilled nursing care in the home setting while coordinating with physicians and the interdisciplinary care team.

Clinical Setting

You will work in patients' homes within a 30-mile radius of the corporate office located at 12660 Coit Rd. Suite 200, Dallas, Texas 75251. This is not a hybrid role; you must be able to commute to the Dallas office and be onsite as required.

Responsibilities

  • Assess and evaluate patient cases and prepare and implement individualized treatment and case management plans
  • Perform accurate OASIS collection and ensure medication profiles remain current
  • Ensure lab values are reported to the physician in a timely manner
  • Provide general nursing care to patients in the home setting
  • Administer prescribed medications and treatments in accordance with physician orders and approved nursing techniques
  • Take vital signs including temperature, pulse, and blood pressure to detect deviations from normal
  • Document nursing history and physical assessments for assigned patients
  • Observe patients, record significant conditions and reactions, and notify supervisors and physicians of patient status and reactions to treatments
  • Maintain awareness of patient comfort and safety needs
  • Respond to life-saving situations based on nursing standards, policies, procedures, and protocol
  • Initiate patient education plans according to individualized patient needs as prescribed by physician and company policy
  • Confer with health team members individually and in conferences to exchange, discuss, and evaluate patient information for planning and coordinating treatment programs
  • Explain procedures and treatments to patients to gain cooperation and understanding
  • Prepare and maintain written case records, reports, and forms
  • Participate in weekly case conferences and case manager meetings
  • Complete all documentation within 24 hours of visits, 7 days per week
  • Participate in quality improvement activities

Required Qualifications

  • Current and valid Texas Registered Nurse (RN) license
  • Minimum 2 years of nursing experience
  • Minimum 1-2 years of hospice experience
  • Current and valid Texas driver's license
  • Proof of current and valid auto insurance
  • Proof of TB skin test result
  • Personally owned automobile that is insured in accordance with state and organization requirements and is in good working order
  • Ability to commute to the Dallas office and be onsite
  • Proficiency with OASIS admissions for home health patients
  • Excellent interpersonal skills and ability to communicate effectively with the Altruist team, patients, and referral sources
  • Impeccable work history with professional references
  • Willingness to see patients in the assigned coverage area
  • Compliance with accepted professional standards and practice
  • Good verbal, written communication, and organization skills
