" Home Health Nurse- Mount Sinai at Home- Part Time Days

Mount Sinai at Home is seeking a Part-Time Home Health Nurse to provide safe, competent quality care to patients in their home settings. As a Registered Professional Nurse, you will deliver patient-centered care based on nursing theory, research, and evidence-based practice, incorporating the principles of Relationship Centered Care.

Clinical Setting

You will work in patients' homes, managing a designated caseload and coordinating care with prescribing providers and interdisciplinary team members.

Responsibilities

Patient Care

• Assess and evaluate patient care needs, applying critical thinking skills in patient care management
• Deliver nursing care per provider orders, including establishing peripheral intravenous access, administering intravenous therapy, collecting laboratory samples, and providing wound care
• Establish individualized, measurable goals in consultation with patients, families, and other healthcare providers
• Educate patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention
• Prioritize all aspects of patient care including teaching, coaching, and planning for post-discharge care
• Communicate effectively and professionally with patients, families, and all members of the interdisciplinary patient care team

Patient Experience

• Consider patients' values, preferences, cultural diversity, and expressed needs in all aspects of care
• Establish and maintain therapeutic relationships with patients and families
• Demonstrate caring, respect, compassion, empathy, and active listening
• Initiate service recovery for patients and families with concerns

Quality and Safety

• Practice safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement
• Implement process improvement strategies based on institutional and evidence-based procedures
• Engage in formal and informal peer and institutional review processes
• Collaborate with interdisciplinary teams on quality improvement projects and evidence-based practice activities
• Identify and avoid high-risk behaviors to provide a safe environment
• Document nursing practice accurately to support quality and performance improvement initiatives

Operations

• Manage a designated caseload to ensure all active patients are making progress toward established care goals
• Coordinate with prescribing providers to ensure patients have necessary services, supplies, and durable medical equipment
• Provide instruction and supervision for Home Health Aides and Personal Care Aides
• Document in an accurate and thorough manner in compliance with health system, regulatory, and legal requirements

Required Qualifications

• Graduation from an accredited nursing program with either a Bachelor's Degree in Nursing (BSN) or Associate's Degree in Nursing (ADN)
• RN who is matriculated in a BSN program with a graduation date within the year is acceptable
• Any Associate's Degree in Nursing (ADN) RN with nursing experience must be approved by the Chief Nursing Officer
• Minimum of two years of experience employed as a Registered Nurse
• Current registration as a Registered Nurse in New York State
• Basic Life Support (BLS) certification from the American Heart Association
• Infusion experience

Compensation

The salary range is $87,692.00 to $131,538.00 annually. Actual salaries depend on experience, education, and operational need. This range does not include bonuses, incentive pay, differential pay, or other forms of compensation or benefits.
