Home Health Nurse- LPN/ RN

Centria Healthcare is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing care in patients' homes. You will deliver skilled, personalized care to pediatric and adult patients with complex medical needs in a relationship-based home care setting.

Clinical Setting

This position involves caring for patients in their homes. Three specific client opportunities are available, all located in Ypsilanti, MI:

Adult Client: An 8-hour night shift (10:00 PM – 6:00 AM) on Tuesday and Saturday for a stable, home-bound adult requiring ventilator, tracheostomy, G-tube, and seizure management. A supportive family is on-site to assist and provide orientation. You must be comfortable with small dogs (secured during shifts).

Pediatric Client (1-year-old): 12-hour shifts available as follows: Days (8:00 AM – 8:00 PM) on Sunday, Wednesday, Thursday, and Saturday; Nights (8:00 PM – 8:00 AM) available 7 days per week. This case involves high-tech pediatric care requiring skilled tracheostomy, ventilator, and G-tube management. The family welcomes both male and female nurses and offers virtual and hospital-based meet-and-greets. A friendly dog is in the home.

Pediatric Client (Infant): 12-hour shifts (9:00 AM – 9:00 PM) available Monday through Sunday. This is a rewarding opportunity caring for a sweet infant transitioning home from the NICU. The focus is on TPN administration, central line (Broviac) care, and G-tube management with no respiratory care required. Key tasks include TPN disconnects in the late afternoon and evening setups. You will work alongside a supportive work-from-home mom in a lively, pet-loving household.

Responsibilities

• Provide skilled nursing care including tracheostomy and ventilator care, medication administration, and catheter care
• Monitor and document patient condition and vital signs
• Assist with activities of daily living (ADLs)
• Communicate with families and care teams
• Maintain safe, high-quality, patient-centered care

Required Qualifications

• Active RN or LPN license in the state of Michigan
• Strong communication and critical thinking skills

Preferred Qualifications

• Home care or private duty nursing experience
• Tracheostomy and ventilator experience (training provided if needed)

Schedule and Compensation

Full-time and part-time shifts are available with day, night, and weekend options. Pay is weekly with optional daily pay available. Compensation is competitive and based on experience.
