Home Health Nurse II Short Hour

Kaiser Permanente is seeking a Home Health Nurse II for a short-hour, on-call position based in Novato, California. In this role, you will provide skilled nursing care to patients in their homes, delivering age-appropriate care in accordance with agency policy, state and federal regulations, and the California Nursing Practice Act.

You will work as an individual contributor with no supervisory responsibilities, though you may supervise and teach LVNs and HHAs as required for patient care. This is a call-in/on-call position with 1 hour per week scheduled, working day shift hours between 8:30 AM and 5:00 PM. Travel is required approximately 15% of the time.

Responsibilities

• Conduct initial comprehensive all-system nursing assessments prior to providing care
• Initiate and revise plans of care, obtaining written physician orders for any changes
• Provide care and services according to the established plan of care
• Perform ongoing systematic assessments focusing on physiologic, psychosocial, cultural, spiritual, and cognitive status, including nutrition, pain management, patient and family education, family involvement, and patient advocacy
• Provide periodic all-system assessments and evaluate patient equipment needs
• Initiate preventative and rehabilitative nursing procedures
• Initiate timely discharges based on patient goals
• Educate and instruct patients, families, and staff as required
• Counsel patients and families in meeting nursing and related needs
• Supervise and teach LVNs and HHAs as required
• Provide nursing care through all phases of care, considering age and developmental needs
• Document all care, interventions, and care coordination per law, regulation, and agency policy
• Notify the patient's attending physician, podiatrist, and other professionals of significant changes in patient condition in a timely manner
• Assist in coordinating all services provided
• Consult with other health professionals when needed
• Effectively plan and organize work assignments, manage priorities, and complete assignments with minimal direct supervision within established timeframes
• Adhere to Kaiser Permanente Medical Care Program policies and principles
• Behave in a courteous, professional manner with patients and health professionals while maintaining patient confidentiality
• Act as a patient advocate in utilizing Medicare, MediCal, health plan, and community resources
• Participate in departmental quality assurance and utilization review activities as requested
• Attend education programs to maintain current knowledge in your area of practice
• Utilize new knowledge and skills in your work
• Participate in in-service programs
• Prepare annual professional goals and review progress with your supervisor

Required Qualifications

• Two years of applicable registered nurse experience within the last five years, with at least six months of home health experience in a hospital-based or community-based home health agency
• Knowledge of the nursing process
• Commitment to service orientation
• Effective written and oral communication skills in English
• Good interpersonal skills
• High school diploma or GED
• Graduate from an approved school of professional nursing (AA, Diploma, or BSN)
• Active California Registered Nurse license
• Valid California driver's license
• Basic Life Support certification

Preferred Qualifications

• Computer literacy
• Experience with automated clinical information systems
• Oncology, hospice, or care of terminally ill experience (if working within the Hospice Department)
