Talin Home Healthcare LLC is seeking compassionate and dedicated Home Health Nurses to provide skilled nursing care to patients throughout Spotsylvania County and the Northern Virginia area. In this role, you will deliver personalized, high-quality care in patients' homes, working independently while building meaningful connections with the families you serve.
Clinical Setting
You will work in patients' homes across the Northern Virginia region, providing direct patient care and collaborating with physicians, therapists, and interdisciplinary care team members.
Responsibilities
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, perform wound care (wet and dry), manage wound vacs, administer IV therapy, provide ostomy care, and deliver other clinical treatments as ordered
- Monitor and document patient progress, vital signs, and changes in condition
- Educate patients and family members on disease management, medications, and home care techniques
- Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
Required Qualifications
- Active RN license in the state of Virginia
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Reliable transportation and valid driver's license for travel throughout the Northern Virginia region
- Ability to work independently and manage a flexible patient caseload
- Minimum commitment of 2 days per week, with one day being Monday and the other being either Wednesday or Friday
Preferred Qualifications
- Previous home health, hospital med-surg, or community nursing experience
Schedule
Minimum 2 days per week with flexible scheduling. One day must be Monday, and the other day must be either Wednesday or Friday.
