Job Description

Talin Home Healthcare LLC is seeking compassionate and dedicated Home Health Nurses to serve patients across Loudoun County and the Northern Virginia area. In this role, you will deliver skilled nursing care in patients' homes, building meaningful relationships while providing high-quality, personalized care. This position is ideal for nurses who thrive in independent environments and are committed to making a direct difference in patients' lives.

Clinical Setting

You will work in patients' homes throughout the Northern Virginia region, providing direct patient care and collaborating with physicians, therapists, and care team members to ensure coordinated, patient-centered care.

Responsibilities

  • Provide skilled nursing care to patients in their homes across the Northern Virginia area
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Administer medications, perform wound care (wet and dry), manage wound vacs, administer IV therapy, provide ostomy care, and deliver other clinical treatments as ordered
  • Monitor and document patient progress, vital signs, and changes in condition
  • Educate patients and family members on disease management, medications, and home care techniques
  • Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
  • Maintain accurate and timely clinical documentation in compliance with regulatory standards

Required Qualifications

  • Active RN license in the state of Virginia
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Reliable transportation and valid driver's license for travel throughout the Northern Virginia region
  • Ability to work independently and manage a flexible patient caseload

Preferred Qualifications

  • Previous home health, hospital med-surg, or community nursing experience

Schedule

You must commit to working a minimum of 2 days per week. One day must be a Monday, and the other day can be either Wednesday or Friday. Flexible scheduling is available.
