Home Health Nurse for Fair Fax County

Talin Home Healthcare LLC is seeking compassionate and dedicated Home Health Nurses to provide skilled nursing care to patients throughout the Fairfax County and Northern Virginia area. This role offers the opportunity to deliver personalized, high-quality care in patients' homes while working independently and building meaningful relationships with the families you serve.

Clinical Setting

You will work in patients' homes across the Northern Virginia region, managing a flexible patient caseload and traveling throughout the area to provide care.

Responsibilities

• Provide skilled nursing care to patients in their homes
• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, perform wound care (wet and dry), manage wound vacs, administer IV therapy, provide ostomy care, and deliver other clinical treatments as ordered
• Monitor and document patient progress, vital signs, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Required Qualifications

• Active RN license in the state of Virginia
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for travel throughout the Northern Virginia region
• Ability to work independently and manage a flexible patient caseload

Preferred Qualifications

• Previous home health, hospital med-surg, or community nursing experience

Schedule

Minimum commitment of 2 days per week, with one day being Monday and the other day being either Wednesday or Friday.
