Home Health LVN (Visiting Nurse) San Gabriel Valley

Care Unlimited Health Services is hiring Licensed Vocational Nurses (LVNs) to provide skilled, one-on-one home health care in the San Gabriel Valley area, including Alhambra, San Gabriel, San Dimas, Glendora, and West Covina. This is a per-visit, field-based position with immediate availability and flexible scheduling options.

Responsibilities

• Prepare, administer, and accurately record medications per physician orders and protocols during home visits
• Perform all duties in compliance with HIPAA guidelines, agency standards, and electronic charting procedures
• Act as the eyes and ears in the home, reporting clinical observations and patient status directly to the Case Manager, RN Supervisor, and Director of Nursing
• Provide skilled, personalized patient care across your assigned territory

Schedule and Compensation

• Work Type: Per-Visit (Field Based)
• Availability: Full-Time or Part-Time options
• Schedule: Weekday and weekend visits available
• Compensation: Competitive pay per visit, $40 to $45 per hour

Travel Requirements

• Daily travel to patient homes within the San Gabriel area is required
• Valid California Driver's License and reliable auto insurance are mandatory

Required Qualifications

• Current California State LVN Licensure
• Current CPR/BLS Certificate
• Current TB/Physical Clearance and Negative TB Test Result
• Valid California Driver's License
• Reliable auto insurance

Preferred Qualifications

• Experience with Electronic Home Health Documentation systems (such as Kinnser)
• Bilingual proficiency in Spanish, Mandarin, or Cantonese
