Aveanna is seeking a full-time Licensed Practical Nurse to deliver skilled nursing care to patients in their homes throughout Orange County. You will work Monday through Friday, providing direct patient care under the direction of a Registered Nurse teammate, implementing physician orders, and maintaining detailed documentation of all care provided.
Clinical Setting
You will provide care in patients' homes, working as part of a home health team. Your territory will be Orange County.
Responsibilities
- Deliver quality patient care in compliance with physician orders under the direction of your Registered Nurse teammate
- Perform hands-on client care including documentation and timely communication with the client's physician and other caregivers
- Recognize changes in patient needs and responses requiring intervention, implementing care to prevent or reduce risk
- Educate clients and their family members based on the client's specific needs
- Provide a variety of skilled nursing services including phone triage, patient education, observation and assessment, wound care, infusions, catheter care, and PICC line dressing changes
Required Qualifications
- Active LPN license in the state of application
- Valid CPR certification
- Valid driver's license
- Proof of automobile insurance
Preferred Qualifications
- One year of LPN experience in a home health or healthcare setting
- Home Care Home Base experience
- IV certification
