Home Health LPN Field Nurse

Stonerise Home Health is seeking a Home Health LPN Field Nurse to provide skilled nursing care to patients in their homes. In this role, you will help patients maintain their independence and dignity while working as part of an interdisciplinary team that includes nurses, therapists, social workers, and home health aides.

Responsibilities

• Follow plans of care and provide treatments as directed by the RN
• Assess patients and provide education to patients and families about their diagnosis and plan of care
• Provide direct patient care in the home setting
• Coordinate home health services and report identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the RN and Clinical Manager
• Maintain accurate and timely documentation
• Use equipment and supplies effectively and efficiently
• Participate in personal and professional growth and development

Required Qualifications

• Active LPN license in Ohio and West Virginia
• Graduate of an accredited practical nurse or vocational nursing program
• Current CPR certification

Preferred Qualifications

• Two years of nursing experience in community/home health or medical/surgical settings
