Home Care Providers of Texas is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in the South Dallas area. In this role, you will deliver compassionate, evidence-based care in patients' homes while working collaboratively with RN Case Managers and physicians to support patient recovery and wellness.
Responsibilities
- Conduct skilled nursing visits in accordance with each patient's individualized plan of care
- Assist the RN Case Manager in developing, reviewing, and updating care plans based on clinical findings and patient needs
- Administer nursing interventions and treatments as outlined in the care plan
- Observe and document patient condition and response to treatments, promptly reporting any changes or concerns to the RN Case Manager and/or physician
- Provide education and support to patients, families, and caregivers regarding the plan of care, treatment procedures, and self-care techniques
Clinical Setting
This is a home health position serving geriatric patients in the South Dallas area. You will work independently in patients' homes with support from a clinical team.
Required Qualifications
- Active LVN License (Texas)
- Active CPR Certificate
- Prior experience in geriatric home health care
- Valid Driver's License
- Self-starter with strong organizational skills
