Home Health Licensed Vocational Nurse (LVN) - North San Antonio

Home Care Providers of Texas is seeking a full-time Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes throughout North San Antonio, TX and surrounding areas. In this role, you will deliver compassionate, evidence-based care while working collaboratively with RN Case Managers and physicians to support patient recovery and wellness in a home health setting.

Responsibilities

• Conduct skilled nursing visits in accordance with each patient's individualized plan of care
• Assist RN Case Managers in developing, reviewing, and updating care plans based on clinical findings and patient needs
• Administer nursing interventions and treatments as outlined in the care plan
• Observe and document patient condition and response to treatments
• Promptly report any changes or concerns to the RN Case Manager and/or physician
• Provide education and support to patients, families, and caregivers regarding the plan of care, treatment procedures, and self-care techniques

Clinical Setting

This is a home health position serving geriatric patients. You will work independently in patients' homes with no office time required. The employer has recently implemented the Roger app to streamline documentation, which can reduce documentation time by approximately 50%, allowing you to focus more on direct patient care.

Required Qualifications

• Active LVN license in Texas
• Active CPR certificate
• Prior experience in geriatric home health care
• Valid driver's license
• Self-starter with strong organizational skills
