CenterWell Home Health is seeking a Licensed Vocational Nurse to provide skilled nursing care to patients in their homes. In this role, you will work under the direction of an RN, delivering personalized, one-on-one care that helps patients regain independence while recovering from injury, illness, surgery, or hospitalization, or while managing chronic conditions.
Reporting to the Clinical Manager, you will be part of a dedicated interdisciplinary team including physicians, clinicians, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers.
Responsibilities
- Provide skilled nursing care to patients in their homes under RN direction
- Implement individualized care plans in collaboration with patients, families, and the healthcare team
- Educate patients and families on disease management, treatment options, and self-care techniques
- Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes
- Maintain prompt contact with patients, physicians, clinical managers, and healthcare team members regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
Required Qualifications
- Current nursing license in the practicing state
- Valid driver's license, auto insurance, and reliable transportation
- Current CPR certification
- Two years of experience as an LPN/LVN in a clinical setting
Preferred Qualifications
- Nursing experience in a Home Health or Hospice setting
Schedule
Compensation
- Pay per visit/unit rate: $33.00 - $47.00
- Annual salary range: $53,000 - $73,000 (based on 40 hours per week, varies by geographic location and individual qualifications)

Additional Requirements
- This role is patient-facing and part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
- This role is part of Humana's driver safety program. You must maintain a valid state driver's license and personal vehicle liability insurance in accordance with your state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
