TheraCare Home Health is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care in home settings under the supervision of a Registered Nurse. You will support patient care delivery, assist with care planning, and collaborate with the interdisciplinary team to ensure high-quality outcomes. This role offers the opportunity to work independently while being part of a supportive clinical team in Beaumont, TX.
Responsibilities
- Provide skilled nursing care to patients in the home setting under RN supervision
- Assist in identifying patient physical, psychosocial, and environmental needs
- Participate in developing and implementing the Plan of Care (POC) in collaboration with the RN
- Administer treatments and interventions in accordance with physician orders and agency policies
- Monitor patient condition and report changes to the supervising RN
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
- Participate in interdisciplinary communication and weekly case conferences
- Provide verbal and written updates to the RN prior to reassessment, supervisory, and discharge visits
- Follow and maintain infection control and safety protocols in accordance with OSHA standards
- Ensure patient safety and quality of care at all times
- Comply with all state regulations and professional standards of nursing practice
- Participate in required in-service training and maintain clinical competency
- Meet productivity expectations and ensure documentation is completed within required timeframes
Required Qualifications
- Active Licensed Vocational Nurse (LVN) license in Texas
- Strong knowledge of patient care, infection control, and documentation practices
- Ability to follow and implement a Plan of Care under RN supervision
- Strong clinical assessment, documentation, and communication skills
- Ability to identify and report changes in patient condition
- Reliable transportation with valid driver's license and current auto liability insurance
- Ability to read, write, and communicate effectively in English
Preferred Qualifications
- Minimum of 1 year of home health experience
- Experience working with interdisciplinary teams in a home health setting
