Home Health Licensed Practical Nurse

Well Care Health is seeking a Licensed Practical Nurse to provide nursing care to patients in their homes. Working under the supervision of a registered nurse, you will assist in implementing individualized care plans for patients across all age groups, from adolescents through geriatrics.

Responsibilities

• Provide direct nursing care in patients' homes, including administering medications, wound care, and other treatments
• Monitor patients' conditions and report changes to the care team
• Assist in assessing, planning, implementing, and evaluating care according to patients' age and diagnosis
• Educate patients and families on health management, medications, and self-care techniques
• Collaborate with the multidisciplinary healthcare team to ensure quality care
• Communicate patient information effectively in accordance with agency policies and discipline-specific guidelines
• Maintain and adjust your schedule to support agency performance
• Demonstrate the agency's mission, vision, and values in daily work
• Maintain positive interpersonal relationships with patients, families, physicians, and staff
• Promote customer satisfaction

Required Qualifications

• Graduate of an accredited or approved school of practical nursing
• Current North Carolina license to practice practical nursing
• CPR certification
• Minimum one year of practical nursing experience
• Valid North Carolina driver's license and an operational vehicle
• Hand/eye coordination for administering injections, using computers, and manipulating patient care equipment
• Ability to communicate and be literate in English
• Ability to transfer and maneuver objects weighing at least 50 pounds
• Ability to assess patient conditions, formulate care plans, select appropriate interventions, and evaluate patient responses
• Ability to explain and teach patients about their condition and recovery
• Ability to visually assess patients and implement care (changing dressings, performing phlebotomy, regulating IVs, maintaining equipment)
• Ability to auscultate lung and bowel sounds and hear alarms
• Ability to prioritize duties and learn new skills and techniques

Preferred Qualifications

• Home health nursing experience

Physical Requirements

• Frequent pushing, moving, and lifting of patients
• Positioning patients, bathing patients, and ambulating patients requiring significant physical effort
• Occasional overhead reaching, stair climbing, and fine motor manipulation
• Ability to work in varying temperatures and weather conditions

Schedule

• Variable Monday through Friday, weekends, and holidays as needed

• Hourly range: $31.75-$35.00 (based on market rates and individual qualifications)
