Home Health Licensed Practical Nurse

Join our home health team as a Licensed Practical Nurse and provide direct patient care in clients' homes. You'll deliver technical nursing services, support patients in learning self-care techniques, and collaborate with case managers and clinical supervisors to ensure quality care and dignified patient experiences.

Responsibilities

• Provide technical nursing services in accordance with agency procedures and LPN licensure regulations
• Prepare clinical and progress notes
• Assist patients in learning appropriate self-care techniques
• Confer with case managers or clinical supervisors regarding changes in patient condition and needs

Schedule

• One-on-one patient care

Required Qualifications

• Graduation from an approved school of Practical Nursing
• Active license to practice as an LPN in the State of Indiana
• One year of nursing experience
• Licensed Practical Nursing curriculum must have included a pharmacology course
• Current CPR certification
• Valid state-issued driver's license
• Automobile insurance and reliable transportation
• Willingness to travel
