Home Health Licensed Practical Nurse - Santa Rosa/Escambia Counties

We are seeking a Licensed Practical Nurse (LPN) to join our home health team serving Santa Rosa and Escambia Counties in the Pensacola area. In this PRN position, you will follow individualized patient care plans created by the RN, providing direct patient care in patients' homes. You will assess and monitor patients for changes in condition, document observations, and communicate effectively with the clinical team to ensure optimal patient outcomes.

Clinical Setting

This is a community-based home health position covering the following service areas:

• Santa Rosa County: Pace, Milton, Gulf Breeze, Navarre
• Escambia County: Cantonment, Pensacola

Responsibilities

• Coordinate with colleagues to assess, evaluate, and follow patient care plans created by the RN in a home health setting
• Assess and monitor patients for abnormalities and changes in status
• Document changes in patient conditions and report observations to your supervisor
• Maintain accurate records of patient care

Schedule

This is a PRN (as-needed) position with a minimum productivity requirement of 10 points per week, where each home visit equals 1 point.

Required Qualifications

• Current Florida LPN license
• Valid driver's license with reliable transportation and active auto insurance
• BLS certification

Preferred Qualifications

• Previous experience as a home health LPN
• IV Therapy Certification
