CenterWell Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes on a PRN basis. You will work under the direction of a Registered Nurse and Clinical Manager, delivering personalized, one-on-one care that helps patients regain independence while recovering from illness, injury, surgery, or hospitalization, or while managing chronic conditions.
Responsibilities
- Provide skilled nursing care to patients in their homes under RN direction
- Implement individualized care plans in collaboration with patients, families, and the healthcare team
- Educate patients and families on disease management, treatment options, and self-care techniques
- Maintain accurate records of patient visits, interventions, and outcomes
- Maintain prompt contact with patients, physicians, clinical managers, and healthcare team members regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
Required Qualifications
- Current nursing license in your practicing state
- Valid driver's license, auto insurance, and reliable transportation
- Current CPR certification
- Two years of experience as an LPN/LVN in a clinical setting
Preferred Qualifications
- Nursing experience in a home health or hospice setting
Schedule and Employment
- PRN (as-needed) position
- Scheduled weekly hours: 1 (PRN basis)
Compensation and Benefits
- Pay range: $53,000 - $73,000 per year (based on full-time 40 hours per week equivalent; actual compensation varies by geographic location and individual qualifications)

Additional Requirements
- This is a patient-facing role and requires TB screening as part of Humana's TB screening program
- Valid state driver's license required; you must maintain personal vehicle liability insurance in accordance with your state's minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage, whichever is higher
- This role is part of Humana's driver safety program
