Home Health Licensed Practical Nurse PRN

CenterWell Home Health is seeking a Licensed Practical Nurse (LPN) on a PRN basis to provide skilled nursing care to patients in their homes. You will work under the direction of a Registered Nurse and Clinical Manager, delivering personalized, one-on-one care that helps patients regain independence while recovering from illness, injury, surgery, or hospitalization, or while managing chronic conditions.

Responsibilities

• Provide skilled nursing care to patients in their homes under RN direction
• Implement individualized care plans in collaboration with patients, families, and the healthcare team
• Educate patients and families on disease management, treatment options, and self-care techniques
• Maintain accurate records of patient visits, interventions, and outcomes
• Maintain prompt contact with patients, physicians, clinical managers, and healthcare team members regarding patient or family needs and status changes
• Participate in care coordination activities and discharge planning

Required Qualifications

• Current nursing license in the practicing state
• Valid driver's license, auto insurance, and reliable transportation
• Current CPR certification
• Two years of experience as an LPN/LVN in a clinical setting

Preferred Qualifications

• Nursing experience in a Home Health or Hospice setting

Schedule and Employment

• PRN (as-needed) position
• Scheduled weekly hours: 1 hour (PRN basis)

Compensation and Benefits

• Pay per visit/unit rate: $33.00 - $47.00
• Annual salary range (full-time equivalent): $53,000 - $73,000 per year

Additional Requirements

• This role is patient-facing and part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
• This role is part of Humana's driver safety program. You must maintain a valid state driver's license and personal vehicle liability insurance in accordance with your state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
