Holy Cross Home Care and Hospice is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing care to clients in their homes throughout Montgomery or Prince George County, Maryland. In this role, you will provide care in accordance with individualized plans of care prescribed by physicians and assigned by supervising registered nurses. This position offers the opportunity to practice at the top of your license with greater flexibility, autonomy, and meaningful one-on-one patient relationships, whether you are experienced in home health or transitioning from hospital, acute care, or skilled nursing settings.
Responsibilities
- Deliver skilled nursing care to home health clients according to physician-prescribed plans of care
- Coordinate with supervising registered nurses on care assignments and clinical decisions
- Maintain efficient documentation using the Epic EHR system
- Build and maintain therapeutic relationships with patients and families
- Provide direct patient care with focus on quality outcomes and patient satisfaction
Clinical Setting
Home-based care across Montgomery and Prince George County, Maryland. Holy Cross Home Care and Hospice is a Medicare-certified, Joint Commission-accredited member of Trinity Health At Home, a national home care, hospice, and palliative care organization. Services include skilled nursing, physical therapy, occupational therapy, speech therapy, and medical social work.

Required Qualifications
- Minimum one year of experience in an acute care setting
- Graduate from an accredited or state-approved school of practical nursing
- Current licensure as an LPN in the State of Maryland
- CPR certification
- Passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications
- Current driver's license and reliable transportation to and from work sites
