Home Health Licensed Practical Nurse (LPN)

Hope Home Care is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their home environment. In this full-time position, you will work one-on-one with patients to deliver education and clinical care that supports their independence and recovery.

Clinical Setting

You will work in patients' homes as part of a supportive team of clinicians. Approximately 15-30% of your 40-hour work week will be spent in the field traveling between patient visits.

Responsibilities

• Participate in planning and coordinating total patient care in conjunction with the RN and physician's plan of care
• Follow the nursing care plan for each patient
• Accurately report and record patient condition and care, including signs and symptoms that may indicate change
• Prepare clinical and progress notes and summaries
• Assist patients with activities of daily living while teaching appropriate self-care techniques
• Follow universal precautions and all emergency procedures when providing patient care

Required Qualifications

• One year of experience as an LPN in home care, medical-surgical, or emergency department setting
• Current state LPN license
• Current CPR certification
• Valid driver's license
• Ability to work a flexible schedule
• Excellent interpersonal skills

Schedule

Full-time, Monday through Friday, 8 AM to 4 PM with flexibility. You will spend 15-30% of your average 40-hour work week in the field traveling to patient homes.

Compensation

• Pay per visit: $31-$35 for routine visits
• Bi-weekly pay: $1,860-$2,100+
• Per visit incentive pay in addition to the minimum 30-visit weekly expectation
• Mileage reimbursement at $0.55 per mile
• Payroll processed bi-weekly (26 pay periods)
