Home Health Licensed Practical Nurse - LPN

We are seeking a Licensed Practical Nurse (LPN) to provide home-based patient care in Collinsville, VA and surrounding areas. In this role, you will deliver personalized, one-on-one nursing care to patients recovering from illness, injury, surgery, or managing chronic conditions. You will work independently while collaborating with RNs, physicians, and interdisciplinary care team members to ensure patients receive comprehensive support within their home environment.

Responsibilities

• Provide home-based nursing care to patients under the supervision of an RN
• Monitor patient condition and obtain vital signs
• Perform routine nursing assessments
• Document patient progress and report changes in condition to RN supervisor
• Assist with implementing individualized patient care plans
• Educate patients and families on medications, treatment plans, self-care, and home safety
• Collaborate with physicians, RNs, CNAs, aides, and interdisciplinary team members to coordinate patient care
• Assist with discharge planning and ensure continuity of care
• Maintain accurate and timely documentation

Required Qualifications

• Graduate of an accredited practical nursing program
• Active LPN license in the state of Virginia
• Minimum of 1 year of LPN experience (home health experience preferred) or completion of office preceptor program
• CPR certification
• TB skin test
• Knowledge of state and federal home health regulations
• Strong clinical judgment, communication, and critical thinking skills
• Ability to work independently in a home setting
