Home Health Licensed Practical Nurse (LPN) – Flexible Schedule

Concierge Home Care is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care in the home health setting, serving patients in the Brandon and East Hillsborough County, Florida area. In this role, you will deliver one-on-one patient care, perform assessments, administer medications and treatments, educate patients and caregivers, and collaborate with the supervising RN and interdisciplinary care team. You will work with a user-friendly EMR system featuring AI-assisted charting to streamline documentation and maximize time spent with patients.

Clinical Setting

You will provide home-based skilled nursing care under the supervision of a Registered Nurse. The coverage area is designed to minimize drive time with clustered scheduling in the Brandon and East Hillsborough County region.

Responsibilities

• Provide skilled nursing care under RN supervision
• Perform patient assessments and assist with care planning
• Administer medications and treatments as ordered by physicians
• Educate patients and caregivers on disease management and care plans
• Monitor patient condition and report changes to the supervising RN
• Collaborate with physicians and interdisciplinary teams
• Complete documentation using AI-assisted charting tools

Schedule

This position offers flexible scheduling with multiple options available: full-time, modified full-time, or PRN. You will create and manage your own daily schedule with no hospital shifts, 12-hour rotations, on-call duties, or nights required. Greater work-life balance is achieved through one-on-one patient care in the home.

Compensation

• Pay-per-visit model: $30-$35 per visit (base rate)
• Average full-time earnings: $55,000-$65,000+ annually

Required Qualifications

• Active, unencumbered Florida Licensed Practical Nurse (LPN) license
• Valid driver's license, reliable transportation, and auto insurance
• CPR certification
• Successful completion of AHCA Level 2 background screening

Travel Requirements

You must have reliable personal transportation and valid auto insurance, as you will be traveling between patient homes in the Brandon and East Hillsborough County area.
