Home Health Licensed Practical Nurse

Beacon Home Health is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes throughout the Jackson, Tennessee area. In this full-time position, you will deliver quality patient care within the scope of practice defined by state nursing laws, working collaboratively with physicians, RNs, and other healthcare team members to support patients and their families.

Responsibilities

• Provide skilled nursing care in patients' homes in accordance with individualized plans of care
• Instruct patients and families in appropriate care techniques and maintain a healthy home environment
• Observe, record, and report patient responses to treatment and changes in condition to supervising nurses and physicians on a daily basis
• Collaborate with physicians and RNs to ensure accurate and timely documentation of care
• Coordinate care with other members of the healthcare team

Required Qualifications

• One year of clinical LPN experience
• Current LPN license in Tennessee
• Current CPR certification
• Valid driver's license and vehicle insurance
• Access to a dependable vehicle or reliable public transportation
• Ability to reliably commute to multiple patient residences daily within your assigned geographic area

Schedule and Travel

• Full-time position
• Must travel to multiple patient homes daily within assigned service area
