Home Health Licensed Practical Nurse

Beacon Home Health is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes throughout the Clarksville, TN area and surrounding communities. In this full-time position, you will deliver quality patient care within the scope of practice defined by state nursing laws, working collaboratively with physicians, RNs, and other healthcare team members.

Responsibilities

• Provide skilled nursing care in patients' homes according to the plan of care
• Instruct patients and families in care procedures and maintain a healthy home environment
• Observe, record, and report patient responses to treatment and changes in condition daily to the supervising nurse and/or physician
• Collaborate with physicians and RNs on the healthcare team
• Document care accurately and promptly
• Maintain the established plan of care without alteration

Required Qualifications

• One year of clinical LPN experience
• Current LPN license in the state where services are rendered
• Current CPR certification
• Valid driver's license
• Vehicle insurance
• Access to a dependable vehicle or public transportation
• Ability to reliably commute to multiple patient residences daily within the assigned geographic area

Schedule and Work Location

• Full-time position
• On-the-road work location requiring travel to multiple patient homes daily
