Home Health Licensed Practical Nurse

CenterWell Home Health is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes. In this role, you will work under the direction of a Registered Nurse, implementing individualized care plans that help patients regain independence while recovering from injury, illness, surgery, or hospitalization, or managing chronic conditions.

You will visit patients in their homes to deliver personalized, one-on-one care as part of a dedicated interdisciplinary team that includes physicians, clinicians, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers.

Responsibilities

• Provide skilled nursing care to patients in their homes under RN direction
• Implement individualized care plans in collaboration with patients, families, and the healthcare team
• Educate patients and families on disease management, treatment options, and self-care techniques
• Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes
• Maintain prompt contact with patients, physicians, clinical managers, and healthcare team members regarding patient or family needs and status changes
• Participate in care coordination activities and discharge planning

Required Qualifications

• Current nursing license in the practicing state
• Valid driver's license, auto insurance, and reliable transportation
• Current CPR certification
• Two years of experience as an LPN/LVN in a clinical setting

Preferred Qualifications

• Nursing experience in a Home Health or Hospice setting

Schedule and Compensation

• 40 hours per week
• Pay per visit/unit rate: $37.00 - $52.00
• Annual salary range: $58,300 - $80,300 (based on geographic location and individual qualifications)

Additional Requirements

• This role is part of Humana's Tuberculosis screening program. If selected, you will be required to be screened for TB.
• This role is part of Humana's driver safety program. You must maintain a valid state driver's license and personal vehicle liability insurance in accordance with your state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
