CenterWell Home Health is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes. You will work under the direction of an RN, implementing individualized care plans that support patients recovering from injury, illness, surgery, or hospitalization, or managing chronic conditions. Working with a dedicated team of physicians and clinicians, you will help patients regain independence and return to the life they love.

Responsibilities

  • Provide skilled nursing care to patients in their homes under the direction of an RN
  • Implement individualized care plans in collaboration with the patient, family, and healthcare team
  • Educate patients and families on disease management, treatment options, and self-care techniques
  • Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes
  • Maintain prompt contact with patients, physicians, clinical managers, and other healthcare team members regarding patient or family needs and status changes
  • Participate in care coordination activities and discharge planning

Required Qualifications

  • Current nursing license in the practicing state
  • Valid driver's license, auto insurance, and reliable transportation
  • Current CPR certification
  • Two years of experience as an LPN/LVN in a clinical setting

Preferred Qualifications

  • Nursing experience in a Home Health or Hospice setting

Schedule

  • 40 hours per week

Compensation

  • Pay per visit/unit rate: $33.00 - $47.00
  • Annual salary range: $53,000 - $73,000 (based on 40 hours per week, varies by geographic location and individual qualifications)

Additional Requirements

  • This role is patient-facing and part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
  • This role is part of Humana's driver safety program. You must maintain a valid state driver's license and personal vehicle liability insurance in accordance with your state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
