Home Health License Practical Nurse (LPN)

Trinity Health At Home – Muskegon is seeking a Home Health Licensed Practical Nurse (LPN) to provide skilled nursing care to clients in their homes. In this role, you will deliver care according to physician-prescribed plans and supervising RN assignments, practicing within the scope of Michigan's LP(V)N/RNA Practice Act and agency policies. This position offers the opportunity to practice at the top of your license with greater flexibility, autonomy, and meaningful one-on-one patient relationships.

Trinity Health At Home – Muskegon is a Medicare-certified, Joint Commission-accredited home care, hospice, and palliative care organization serving communities across 11 states. The agency provides skilled nursing, physical, occupational and speech therapy, and medical social work services to patients in their homes.

Responsibilities

• Provide skilled nursing care in accordance with the plan of care prescribed by the physician
• Coordinate care activities with the supervising registered nurse
• Perform nursing activities in compliance with state practice regulations and agency policies
• Build and maintain therapeutic relationships with clients and families
• Document care efficiently using the Epic EHR system

Required Qualifications

• Graduate from an accredited or state-approved school of practical nursing
• Current licensure as an LPN in the State of Michigan
• CPR certification
• Minimum of one year of experience in an acute care setting
• Current driver's license and reliable transportation to and from work sites
