Home Health Field RN - Registered Nurse PRN

This position provides registered nursing care to patients in their homes, delivering comprehensive assessments, treatment planning, and clinical management for individuals with complex medical conditions. As a Home Health Field RN, you will work independently and collaboratively with interdisciplinary teams, patients, and families to promote health, prevent complications, and support recovery or chronic disease management in a patient-centered home environment.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs
• Apply admission and re-certification criteria to establish appropriate levels of care and patient eligibility
• Develop, implement, and document individualized plans of care; recommend revisions as necessary to ensure quality and continuity
• Educate patients and families on self-care techniques, nutrition, and dietary needs
• Determine the scope and frequency of services based on patient acuity and family needs
• Assess caregiver ability to meet patient needs upon admission and throughout the care episode
• Initiate preventative and rehabilitative nursing procedures
• Apply infection control and standard precautions in all patient care activities
• Provide clinical direction to Home Health Aides and LPN/LVNs to ensure quality service delivery
• Prepare clinical and progress notes demonstrating progress toward established goals
• Communicate changes in patient status and intervention outcomes to physicians and other healthcare personnel
• Monitor assigned cases for compliance with third-party payor requirements
• Maintain effective communication with patients, families, team members, and healthcare professionals

Required Qualifications

• Current RN license
• Valid driver's license
• CPR certification
• Valid auto insurance

Preferred Qualifications

• At least 2 years of home health RN experience
• HomeCare HomeBase experience
