Home Health Field Registered Nurse (RN)_Grandbury, Texas

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care to patients in their homes in the Grandbury, Texas area. In this role, you will deliver high-quality clinical care, build relationships with patients and families, and work with an interdisciplinary team to support patients' health goals.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families

Required Qualifications

• Current RN license in the state of Texas
• One or more years of nursing experience (home health experience preferred but not required)
• Strong clinical assessment and critical-thinking skills
• Reliable transportation, valid driver's license, and proof of auto insurance
• Comfortable using EMR/mobile charting
