Home Health Field Registered Nurse (RN) – Weekend Schedule

Choice Health at Home is seeking an experienced Home Health Field Registered Nurse (RN) to provide high-quality, patient-centered care in patients' homes. This position works four 10-hour shifts per week within the Thursday–Monday period, with Saturday and Sunday required. The schedule provides consistent weekend coverage while allowing three days off each week.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs
• Evaluate patient eligibility and establish the appropriate level of home health services
• Develop, implement, and update individualized plans of care
• Educate patients and families about disease management, medications, nutrition, safety, and self-care techniques
• Determine the frequency and scope of services based on patient acuity and caregiver support
• Perform preventive and rehabilitative nursing interventions
• Follow infection-prevention practices and standard precautions
• Supervise and provide clinical guidance to Home Health Aides and LPN/LVNs
• Document patient progress and clinical outcomes accurately and promptly
• Communicate changes in patient condition to physicians and interdisciplinary team members
• Maintain compliance with payer requirements, agency policies, and quality standards

Required Qualifications

• Current and unrestricted RN license
• Valid driver's license
• Current CPR certification
• Valid automobile insurance

Preferred Qualifications

• Minimum of two years of home health RN experience
• HomeCare HomeBase experience

Schedule

• Four 10-hour shifts per week
• Thursday–Monday schedule, always including Saturday and Sunday
• Three days off per week

Compensation and Benefits
