Job Description

21st Century Home Health Services is seeking a Field Registered Nurse (RN) to provide direct patient care in home health settings across the Richmond-Alameda areas. This entry-level position is designed to develop fundamental nursing skills within home health while gaining understanding of operational goals and efficiencies. The role serves as a foundational opportunity for registered nurses aspiring to advance into case management and works under the direction of a Case Manager Registered Nurse.

Clinical Setting

You will visit patients in their homes across the Richmond-Alameda coverage area, providing skilled nursing care to patients with varying acuity levels. 21st Century Home Health Services is the largest home health agency in San Francisco and the fastest-growing in the Bay Area, currently caring for more than 4,000 patients across multiple counties.

Responsibilities

  • Provide hands-on patient care including PICC lines, wound vacs and care, kangaroo and feeding pumps, management of NG, G, and J tubes, staple and suture removal, drains (JP, biliary, nephrostomy), Foley and suprapubic catheters (insertion, removal, and troubleshooting), PleurX drainage systems, trach care, blood draws, ostomy management, SQ and IM injections, IV antibiotic and TPN administration, and compression wraps
  • Provide direct patient care as defined in the California Nurse Practice Act
  • Implement plans of care initiated and directed by the Case Management Registered Nurse
  • Provide accurate and timely documentation within 24-48 hours consistent with the plan of care
  • Assess and provide patient and family/caregiver education pertinent to diagnosis and plan of care
  • Assist in performing skilled needs procedures, preparing equipment and materials for treatments, and helping patients learn appropriate self-care techniques
  • Participate in coordination of home health services, reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the Case Management RN
  • Use equipment and supplies effectively and efficiently
  • Retrieve and use communication on assigned patients to establish priorities of care
  • Provide timely and pertinent reports to the Case Manager
  • Record pertinent and concise information reflecting patients' needs, problems, capabilities, limitations, and response to nursing interventions
  • Report any incidents with bearing on patient care
  • Report pertinent information timely to the Case Management Registered Nurse and provider
  • Attend weekly case conferences as required to coordinate and communicate patient care and treatment information
  • Maintain patient privacy in medical records and when providing care
  • Maintain professionalism and seek opportunities to enhance clinical knowledge and skills
  • Maintain productivity standards as directed by the Case Management Registered Nurse
  • Complete and review start of care assessments, documentation, and plans of care with Case Manager as needed
  • Participate in on-call duties as required by weekend rotation

Schedule

Full-time position, Wednesday through Sunday visits. On-call duties required as part of weekend rotation.

Required Qualifications

  • One year of recent nursing experience (experience requirement may be waived for internal candidates and new graduates)
  • Graduate of an accredited school of nursing with a Bachelor's degree
  • Current California Registered Nurse License (RN)
  • Current Basic Life Support (BLS) Card
  • Valid California Driver's License with acceptable driving record
  • All licenses and certifications must be current at time of hire and sustained throughout employment
  • Self-directed, critical thinker with outstanding interpersonal skills
  • Organized with computer literacy
  • Excellent observation, verbal, and written communication skills
  • Problem-solving skills and basic math skills
  • Nursing skills per competency checklist
  • Must maintain current with annual compliance training and certifications

Compensation and Benefits

Estimated wage range: $130,000 to $160,000. Starting pay is based on relevant experience above the minimum requirements. Compensation follows a pay-per-visit model, with the wage provided as an estimate. 21st Century offers comprehensive health benefits and a 401(k) plan with up to 4% match.

Additional Requirements
