Home Health Field Nurse (RN/LPN) – West Indianapolis, IN

Elite Home Healthcare is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide skilled nursing care in patients' homes throughout Southwest Indianapolis, Clayton, and surrounding Central Indiana communities. As a Home Health Field Nurse, you will deliver one-on-one patient care in the home setting, allowing you to build meaningful relationships with patients and families while focusing on individual patient needs. Full-time and part-time positions are available based on current patient needs and the Plan of Care.

Clinical Setting

You will work independently in patients' homes throughout Southwest Indianapolis, Plainfield, Clayton, Avon, Danville, Mooresville, Camby, and surrounding Hendricks County and Central Indiana communities within Elite's service area.

Responsibilities

• Assess and document the patient's condition
• Review medications and vital signs
• Administer physician-prescribed medications as ordered and permitted
• Provide skilled nursing care according to the patient's Plan of Care
• Educate patients and families on home healthcare procedures and strategies
• Coordinate care with physicians and other members of the patient's healthcare team
• Communicate effectively with the interdisciplinary care team
• Complete accurate and timely clinical documentation
• Report relevant changes in patient condition
• Follow patient-safety, infection-control, privacy, and Elite Home Healthcare procedures

Required Qualifications

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license authorizing practice in Indiana
• Strong clinical nursing skills
• Excellent written and verbal communication skills
• Detail-oriented and organized
• Effective time-management skills
• Comfortable working independently in patients' homes
• Ability to understand and follow the patient's Plan of Care and applicable clinical orders
• Completion of applicable Elite Home Healthcare employment, credentialing, screening, and compliance requirements

Preferred Qualifications

• Home health nursing experience
• Private duty nursing experience
• Community-based nursing experience
• Related nursing experience

Schedule

Full-time and part-time hours available based on patient needs. Patient assignments, schedules, and available hours vary based on individual patient needs and the Plan of Care. Opportunity for additional patient hours based on availability.
