Home Health Field Nurse (RN/LPN) – Carmel, IN | Current Patient Needs

Elite Home Healthcare is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide skilled nursing care in patients' homes throughout Carmel, Arcadia, and surrounding Hamilton County communities in Indiana. This position offers one-on-one patient care with flexible full-time and part-time scheduling, allowing you to build meaningful relationships with patients and families while delivering care in the home setting.

Clinical Setting

You will work independently in patients' homes, providing skilled nursing services according to each patient's individualized Plan of Care. Patient assignments, schedules, available hours, and required nursing credentials vary based on individual patient needs.

Responsibilities

• Assess and document the patient's condition
• Review medications and vital signs
• Administer physician-prescribed medications as ordered and permitted
• Provide skilled nursing care according to the patient's Plan of Care
• Educate patients and families on home healthcare procedures and strategies
• Coordinate care with physicians and other members of the patient's healthcare team
• Communicate effectively with the interdisciplinary care team
• Complete accurate and timely clinical documentation
• Report relevant changes in patient condition
• Follow patient-safety, infection-control, privacy, and Elite Home Healthcare procedures

Required Qualifications

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license authorizing practice in Indiana
• Strong clinical nursing skills
• Excellent written and verbal communication skills
• Detail-oriented and organized
• Effective time-management skills
• Comfortable working independently in patients' homes
• Ability to understand and follow the patient's Plan of Care and applicable clinical orders
• Completion of applicable Elite Home Healthcare employment, credentialing, screening, and compliance requirements

Preferred Qualifications

• Home health nursing experience
• Private duty nursing experience
• Community-based nursing experience
• Related healthcare experience

Schedule and Employment

Full-time and part-time hours are available based on current patient needs. Opportunities for additional patient hours may be available based on availability.
