Home Health Field Licensed Vocational Nurse (LVN)_Godley, Texas

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes in the Godley, Texas area. You will work under the supervision of an RN Case Manager, delivering compassionate care while maintaining clinical independence within a supportive team environment.

Responsibilities

• Provide skilled nursing care under RN Case Manager supervision
• Perform wound care, medication administration, and vital sign monitoring
• Conduct patient assessments and promptly report changes in condition
• Provide disease process teaching and assist with care plan implementation and updates
• Collaborate with physicians, RNs, and other team members to ensure quality care
• Maintain timely and accurate documentation in the electronic medical record (EMR)
• Travel to patient homes within an assigned service area

Required Qualifications

• Current Texas LVN license
• One or more years of nursing experience (home health experience preferred but not required)
• Strong assessment, communication, and clinical skills
• Reliable transportation, valid driver's license, and auto insurance
• Familiarity with EMR and mobile charting
