Home Health Field Licensed Vocational Nurse (LVN)_Dublin, Texas

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes throughout the Dublin, Texas area. This role offers the opportunity to work independently while supported by a strong clinical team, helping patients maintain safety, health, and independence in their own homes.

Responsibilities

• Provide skilled nursing care under the supervision of an RN Case Manager
• Perform wound care, medication administration, vital signs monitoring, and disease process teaching
• Conduct patient assessments and report changes in condition promptly
• Assist with care plan implementation and updates
• Collaborate with physicians, RNs, and other team members to ensure quality care
• Maintain timely, accurate documentation in the EMR
• Travel to patient homes within an assigned service area

Required Qualifications

• Current Texas LVN license
• One or more years of nursing experience (home health experience preferred but not required)
• Strong assessment, communication, and clinical skills
• Reliable transportation, valid driver's license, and auto insurance
• Familiarity with EMR/mobile charting
