Kona Medical Consulting and The Home Health Team is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes across various Texas service areas. This position offers the opportunity to work independently while supported by a strong clinical team, delivering direct patient care in a patient-centered home health setting.
Responsibilities
- Provide skilled nursing care under the supervision of an RN Case Manager
- Perform wound care, medication administration, vital signs monitoring, and disease process teaching
- Conduct patient assessments and report changes in condition promptly
- Assist with care plan implementation and updates
- Collaborate with physicians, RNs, and other team members to ensure quality care
- Maintain timely, accurate documentation in the electronic medical record (EMR)
- Travel to patient homes within an assigned service area
Required Qualifications
- Current Texas LVN license
- One or more years of nursing experience (home health experience preferred but not required)
- Strong assessment, communication, and clinical skills
- Reliable transportation, valid driver's license, and auto insurance
- Familiarity with EMR and mobile charting
