Home Health Coordinator - Registered Nurse

Sentara Home Care is seeking a Registered Nurse to serve as a Home Health Coordinator in Williamsburg, VA. In this full-time, day-shift position, you will coordinate and supervise all nursing care for home health participants according to program standards. You will conduct initial and periodic assessments, administer nursing care to ill, injured, convalescent, or disabled individuals, and advise participants on health maintenance and disease prevention. You will actively participate in interdisciplinary team meetings and interact with facility staff to ensure comprehensive care delivery.

Responsibilities

• Coordinate and supervise all nursing care for participants according to program standards
• Conduct initial and periodic health assessments
• Administer nursing care and medications as needed
• Advise participants on health maintenance and disease prevention
• Actively participate in interdisciplinary team meetings
• Interact with other facility staff to support patient care

Schedule

• Full-time position
• Day shift (First shift)
• Mileage reimbursement provided for all miles driven between patient visits

Required Qualifications

• Associate's Degree in Nursing
• Active Registered Nurse (RN) license in Virginia
• Basic Life Support (BLS) certification (must be obtained within 90 days of hire)
• Minimum one year of nursing experience

Preferred Qualifications

• Home health nursing experience

Compensation and Benefits

• Base pay range: $87,214.40 - $126,900.80 annually (based on full-time employment of 2,080 hours per year)
• Additional compensation may be available, including shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities
