The opportunity

Job Description

Residential Home Health and Hospice is hiring a Clinical Resource Nurse to deliver skilled nursing care to patients in their homes in the Mount Carmel, PA area. This is a full-time position with a $10,000 first-year success bonus for accomplishing key achievements during your first year.

Responsibilities

  • Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team
  • Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders
  • Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported
  • Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation
  • Initiate evaluations to other disciplines when indicated
  • Travel to patient homes within assigned territory

Schedule

  • 40 hours per week
  • Approximately 20% of your day spent driving to and from patient locations

Required Qualifications

  • Active Registered Nurse (RN) license in Pennsylvania
  • One year of RN experience
  • Current driver's license and ability to spend approximately 20% of your day driving to and from patient locations

Additional Requirements
