Home Health Care - Registered Nurse

Havenova Home Care Solutions is seeking a Registered Nurse (RN) to provide clinical oversight, care planning, and Medicare compliance support across home care operations. This role focuses on ensuring high-quality patient care, accurate documentation, and adherence to regulatory standards while working closely with caregivers, families, and healthcare professionals to coordinate care.

Responsibilities

• Conduct patient assessments and develop individualized care plans
• Review and maintain accurate clinical documentation in compliance with regulatory standards
• Coordinate care with caregivers, physicians, case managers, and families
• Monitor patient condition and document changes appropriately
• Ensure care delivery aligns with care plans and clinical needs
• Support Medicare and state compliance requirements through proper documentation and oversight
• Perform supervisory visits and quality assurance checks
• Participate in chart audits and compliance reviews
• Provide clinical guidance to caregivers and staff

Required Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• Valid driver's license and reliable transportation
• CPR certification

Preferred Qualifications

• Home care, home health, or case management experience
• Strong knowledge of clinical documentation and compliance standards
• Experience with Medicare guidelines
