Home Health Admissions Nurse

This position provides professional nursing services to patients receiving home health care. As a Home Health Admissions Nurse, you will work collaboratively with primary care physicians to assess patients, develop individualized plans of care, and educate patients and their families. You will perform initial admission visits, complete comprehensive assessments, and coordinate ongoing skilled nursing services in patients' homes.

Clinical Setting

You will provide nursing care in patients' private residences, traveling to various work sites throughout the service area. The role requires a personal vehicle for routine patient visits. You may encounter homes that are not handicap accessible, and you should be prepared for possible exposure to communicable diseases.

Responsibilities

• Perform comprehensive initial assessments including medical history, home safety evaluations, environmental assessments, and socio-psychological evaluations
• Conduct physical examinations and review all body systems, documenting findings in clinical notes
• Develop individualized Plans of Care (POC) for physician approval and implementation
• Perform OASIS assessments and skilled nursing documentation
• Regularly re-evaluate patients' nursing needs and revise care plans as needed
• Assess patients' Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (iADL) abilities
• Determine medical necessity for additional services and coordinate appropriate professional services
• Supervise Home Health Aides (HHA) in accordance with state and federal requirements and document supervision activities
• Communicate with physicians verbally and in writing regarding treatment and rehabilitative therapy modalities
• Oversee coordination of patient services and inform physicians and staff of changes in patient status or needs
• Prepare and submit timely clinical and progress notes
• Review billing processes with patients and families, advising them regarding co-pays and insurance coverage
• Maintain effective ongoing communication with patients, families, supervisors, and interdisciplinary team members
• Submit timely requests for visit re-authorization to payers
• Coordinate community services to support safe home care
• Participate in case conferencing sessions and performance improvement committee processes
• Conduct clinical record reviews per agency policy in collaboration with the Nursing Supervisor
• Participate in staff meetings and in-service training
• Serve as a preceptor for nursing staff as needed
• Participate in planning, operation, and evaluation of nursing services
• Notify the agency promptly of emergencies, illness, or other occurrences affecting patient caseload
• Submit written time-off requests at least two weeks in advance
• Perform other duties as assigned

Required Qualifications

• Graduate of an approved professional nursing program (RN)
• Current Massachusetts RN license in good standing
• Minimum one year of professional nursing experience
• Current CPR certification
• Successful criminal background check

Preferred Qualifications

• Prior experience in home care nursing

Physical Requirements

On a daily basis, you will be required to walk, stand, sit, reach, use a telephone, use computers and office equipment, lift or transfer 150 pounds or greater, bend, stoop, and climb stairs. You must be able to access patients' homes that may not be handicap accessible and must be able to hear, speak, understand, and communicate effectively in English.

Special Equipment

You will operate a personal vehicle for patient visits and may use transfer or lifting devices and medical supplies as part of patient care.
