Home Health Admissions & Intake Nurse- Des Moines Area

As a Home Health Admissions & Intake Nurse with Iowa Home Care, you will conduct thorough initial assessments of new patients and develop personalized care plans that enable them to receive high-quality, compassionate care in their own homes. Working closely with physicians and the interdisciplinary healthcare team, you will assess patient conditions, create individualized care plans, and advocate for patient-centered care while educating families and support staff.

Responsibilities

• Conduct thorough initial assessments of patients to establish individualized care plans in collaboration with the physician
• Develop, implement, and adjust care plans based on patient needs, ensuring all aspects of care meet high clinical standards
• Communicate and coordinate with families, therapists, social workers, and home health aides to ensure a well-informed care team
• Advocate for patient needs and provide ongoing education to families and caregivers, empowering them to support the patient's care effectively
• Document all assessments, care plans, and updates accurately and timely in accordance with company policies

Required Qualifications

• Current RN licensure in Iowa
• Valid driver's license and reliable transportation required for travel to client homes
• Strong assessment, organizational, and communication skills
• Ability to work collaboratively with a diverse healthcare team and demonstrate empathy toward clients and their families

Preferred Qualifications

• Prior experience in home health or healthcare admission
