This position provides comprehensive nursing care for a designated group of patients in a home care setting in Queens. As a Home Care Registered Nurse, you will use the nursing process to assess, diagnose, plan, implement, and evaluate individualized patient care while collaborating with physicians and interdisciplinary health team members.
Responsibilities
- Conduct patient interviews, explain policies and procedures, review patient charts, and answer questions courteously
- Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status; determine mobility, sensory deficits, prostheses use, and skin condition
- Assess pain levels and pain management effectiveness
- Assess patient psychosocial health status, coping mechanisms, knowledge level, and ability to comprehend; identify cultural and ethnic requirements
- Identify patient outcomes and develop criteria for measurement; identify actual and potential patient problems
- Establish priorities and coordinate cost-effective use of supplies, equipment, and medication
- Implement the plan of care while maintaining constant vigilance over patient safety
- Administer prescribed treatments correctly and safely using appropriate equipment
- Provide emotional support to patients and significant others
- Maintain accurate documentation of patient treatment, activity, condition, and responses to interventions
- Act rapidly and effectively during emergency situations while maintaining a calm atmosphere
- Participate in discharge planning, coordinate referrals, and provide discharge instructions
- Perform Grade I-IV decubitus care and preventive skin care measures
- Apply simple dressings using aseptic technique; apply warm and cold compresses and perform irrigations per facility procedure
- Administer medications correctly and safely; educate patients and significant others regarding drug and food interactions
- Participate in patient and family education; recognize and utilize health teaching opportunities
- Perform related duties as required
Required Qualifications
- Graduate from an accredited School of Nursing
- Current License to practice as a Registered Professional Nurse in New York State
- Valid Driver's License in New York State (may be required depending on area of assignment)
- Enrollment in an accredited BSN program within two years of job entry date
- Obtain a BSN Degree within five years of job entry date
