Home Care RAI Visits - Registered Nurse or Licensed Practical Nurse

BAYADA/Integrity Home Care is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to conduct annual Resident Assessment Instrument (RAI) and Global Health Evaluation (GHE) assessments for Medicaid clients receiving in-home services in Greene County, Missouri and surrounding counties. This is a part-time, visit-based position with flexible scheduling and no nights, weekends, holidays, or on-call duties.

In this role, you will work one-on-one with patients in their homes and communities to help identify their strengths, needs, and overall health status. Your assessments support the State of Missouri in determining continued Medicaid eligibility and reauthorizing in-home services, ensuring clients receive individualized care that supports their independence, health, and well-being.

Responsibilities

• Conduct annual RAI and GHE assessments with Medicaid clients receiving in-home services
• Evaluate functional abilities, health status, and care needs
• Accurately document findings and maintain compliance with state requirements
• Collaborate with clients, caregivers, and interdisciplinary teams
• Support the reauthorization process for Medicaid-funded services
• Monitor changes in client status and identify potential care needs

Schedule

• Part-time visits: 15-45 monthly RAI visits
• Flexible scheduling

• Mileage paid
• Pay per visit (PPV) compensation model

Required Qualifications

• Current Missouri RN or LPN license in good standing
• Current CPR certification
• Strong organizational and communication skills
