Vanderbilt Health is seeking a Licensed Practical Nurse for intermittent home care. In this role, you will work collaboratively with registered nurses and healthcare team members to provide direct patient care in home settings. You will care for both pediatric and adult patients with a variety of clinical needs, including chronic disease management, post-surgical recovery, wound care, medication management, and patient education.
This position offers a unique opportunity to practice nursing with high levels of independence and autonomy while maintaining strong clinical skills and critical thinking. You will build meaningful relationships with patients and families, coordinate care with interdisciplinary teams, and help individuals safely remain at home while reducing hospitalizations.
Schedule
- Monday through Friday, 8:00 a.m. to 4:30 p.m.
Responsibilities
- Collaborate with patients, families, and healthcare providers in delivering health care services
- Partner with the healthcare team by following the established plan of care to ensure patient progress and effective outcomes
- Support plans to promote health and maintain a safe environment
- Navigate medical scenarios with autonomy under the direction of the supervising RN while maintaining compliance with physician-ordered plans of care
- Administer intravenous fluids and perform sterile procedures
- Recognize and respond to clinical situations appropriately and timely to prevent patient decompensation and injury
- Identify and assess patient needs and clinical changes, accurately reporting assessment findings in a timely manner to the supervising RN and physician
- Provide wound care, point-of-care testing, vital signs monitoring, and other nursing interventions
- Educate patients and caregivers on health management and disease prevention
- Maintain accurate and timely patient documentation
Required Qualifications
- Active Licensed Practical Nurse license in Tennessee
- Minimum one year of relevant nursing work experience
- Graduate of an approved discipline-specific nursing program
