AdventHealth is seeking a Heart Failure Clinic Nurse Practitioner to provide comprehensive clinical care to patients with heart failure and related cardiovascular conditions. This full-time, day-shift position is based in Davenport, Florida.
In this role, you will conduct comprehensive health assessments including physical examinations, patient histories, and interpretation of diagnostic tests. You will initiate orders for medications, laboratory studies, diagnostic testing, and treatments within your scope of practice and organizational policy. You will diagnose and manage acute and chronic health conditions, develop individualized plans of care considering age-specific needs, and provide patient and family education regarding treatment plans and health management.
You will work collaboratively with physicians and interdisciplinary team members to coordinate patient care, participate in interdisciplinary rounds and case conferences, and facilitate efficient patient flow and discharge planning. You will respond to urgent or emergent patient needs and participate in on-call responsibilities as scheduled. You will ensure accurate and timely documentation of patient encounters, procedures, and care plans, and participate in quality improvement activities and evidence-based practice initiatives.
Responsibilities
- Conduct comprehensive health assessments, including physical examinations, patient histories, and interpretation of diagnostic tests
- Initiate orders for medications, laboratory studies, diagnostic testing, and treatments within scope of practice and organizational policy
- Identify alterations in patient health status and implement appropriate nursing and medical interventions
- Diagnose and manage acute and chronic health conditions across organ systems
- Develop, implement, and evaluate individualized plans of care considering age-specific needs
- Provide patient and family education, counseling, and support regarding treatment plans and health management
- Coordinate patient care in collaboration with physicians and interdisciplinary team members
- Facilitate efficient and effective patient flow, including appropriate utilization of resources and discharge planning
- Participate in interdisciplinary rounds, case conferences, and care planning activities
- Ensure accurate and timely documentation of patient encounters, procedures, and care plans in accordance with policy
- Respond to urgent or emergent patient needs and participate in on-call responsibilities as scheduled
- Participate in quality improvement activities, including data collection, outcome analysis, and process evaluation
- Incorporate evidence-based practice to enhance clinical outcomes and support program development
- Engage in professional development activities and represent the organization in appropriate community or educational events
Required Qualifications
- Master's degree in Nursing
- Advanced Practice Registered Nurse (APRN) license
- One or more years of related work experience
- Strong interpersonal skills, excellent conflict resolution, and communication skills
- Leadership, mentorship, and clinical resource skills
- Ability to multi-task on a variety of tasks and projects
- Excellent organization skills
- Skilled in providing age-appropriate education regarding care management interventions and illness adjustment issues
- Ability to use specialized equipment used in the respective area of practice
Preferred Qualifications
- Basic Life Support - CPR Certification (BLS)
Licenses and Certifications
- Advanced Practice Registered Nurse (APRN) license is required
- Additional licensure or certification requirements may apply depending on the specific unit or state in which this position is located
- Certain positions are subject to Florida Level 2 background screening, including fingerprinting, as required by state law
