Full Time Registered Nurse | Horizon Home Health | RN-Fruitland Idaho

Horizon Home Health & Hospice is seeking a Full Time Registered Nurse for home health services in the Fruitland, Idaho area. This position offers the independence and autonomy that home health nursing provides, with the opportunity to deliver comprehensive care to patients in their homes while coordinating with physicians and the interdisciplinary care team.

Responsibilities

Patient Care:

• Complete initial assessments of patients and families to determine home care needs, including comprehensive assessment and history of current and previous illnesses
• Regularly re-evaluate patient nursing needs
• Initiate and revise plans of care as patient status and needs change
• Use health assessment data to determine nursing diagnoses
• Develop care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions, including patient and family in the planning process
• Initiate appropriate preventive and rehabilitative nursing procedures
• Administer medications and treatments as prescribed by the physician
• Counsel patients and families in meeting nursing and related needs
• Provide health care instructions to patients as appropriate per assessment and plan of care
• Identify discharge planning needs and implement prior to patient discharge
• Serve as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload
• Adhere to the organization's documentation and care procedures and standards of personal and professional conduct

Communication:

• Prepare clinical notes and update the primary physician when necessary and at least every sixty days
• Communicate with physicians regarding patient needs and report any changes in patient condition; obtain and receive physician orders as required
• Communicate with community health-related persons to coordinate the care plan

Additional Duties:

• Participate in on-call duties as defined by the on-call policy
• Ensure arrangements for equipment and other necessary items and services are available

Required Qualifications

• Active RN license in Idaho or Oregon
• Valid driver's license and auto insurance

Preferred Qualifications

• Home health experience
