Full-Time Nurse Practitioner - Geriatric Care - New Grads Welcome

This organization delivers primary care to geriatric patients in their homes and long-term care facilities throughout the Akron area. As a Nurse Practitioner in Geriatric Primary Care, you will serve as the primary care provider for a dedicated panel of patients, conducting house calls and facility-based visits.

Clinical Setting

Patients are seen in private homes, assisted living facilities, and independent living facilities. The practice uses a mobile-based EHR with AI functionality to reduce administrative time. Dedicated schedulers coordinate daily routes and schedules to ensure efficiency and appropriate time for each visit.

Responsibilities

• Conduct comprehensive health assessments to identify patient needs
• Develop personalized care plans for each patient
• Regularly evaluate patient conditions and responses to treatment
• Adjust treatment plans as necessary based on patient progress
• Perform minor procedures and wound care as required and as settings permit
• Educate patients and families on health management and disease prevention
• Provide guidance on lifestyle modifications and self-care practices
• Coordinate care with physicians, nurses, and other healthcare professionals

Schedule

• Full-time position
• 4-day or 5-day schedule options
• No evenings, weekends, or holidays
• Minimal on-call duties: phone triage only, typically one night per month

Compensation and Benefits

• Annual income: $125,000 to $170,000
• Base salary: $105,000 to $125,000
• Annual bonus: $20,000 to $45,000, paid quarterly

Required Qualifications

• Active Ohio NP license
• DEA registration

Akron, Ohio. The practice will work with providers during the interview process to establish a mutually agreeable geographic coverage area.
