Full Time - Licensed Vocational Nurse (LVN) for Home Health - Junction, TX

Hill Country Senior Care Home Health is seeking a Licensed Vocational Nurse (LVN) to provide nursing care to patients in their homes in Junction, TX. Working under the direction of a registered nurse or physician, you will deliver direct patient care, manage medications and treatments, monitor vital signs, and coordinate with the healthcare team to ensure quality outcomes.

Responsibilities

• Assess patients' health status and develop care plans
• Administer medications and treatments as prescribed by a physician
• Monitor patients' vital signs and report any changes to the RN or physician
• Provide wound care and other nursing interventions as needed
• Document patient care and progress in electronic medical records
• Communicate with patients, families, and other healthcare providers as needed
• Participate in interdisciplinary team meetings and case conferences
• Adhere to all state and federal regulations and agency policies and procedures
• Perform wound care 2-3 times per week

Required Qualifications

• Current LVN license in the state of Texas
• Current CPR certification
• Minimum of one year of experience in home health care
• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong critical thinking and problem-solving skills
